BACKGROUND
In 2015 UCSF and JMH partnered to create a Bay Area-wide accountable care network to grow our footprint beyond Contra Costa County and attract more patients. A year later, Canopy Health was born and so was the vision to create an integrated patient experience while leveraging multiple EMRs.
That is when I was asked to join a team with representatives from both sides, to lead the design effort for their new website and new patient portal.
PROBLEM
How to create an integrated patient experience while leveraging multiple EMRs and combining plan data with clinical data. The only success story on the market is that of Kaiser.
SOLUTION
Create a single depository for members' preferences. Establish live-connections to external systems.

Define user behavior and system interactions. Design information architecture, interaction and visual design.
