This is a composite of the diffuse map with simulated gold foil and embossing for the sake of this presentation. For the label on the bottle in Modo, actual highlights and shadows of the embossed elements are achieved via displacement map. Gold foil reflections are properties of the Modo's shader; they're unique to the lighting and environment of the 3D scene.
In Illustrator, I arranged every group element of the label in separate layers. Highlights and embossed elements are also isolated. I then exported individual layers to Photoshop for painting and separations.
I hand-lettered the script in Illustrator. The font was difficult to identify and I suspect it is custom. No worries. Here's a portion of my drawing.
