Redesigning Salmon Samurai's Order Form

Redesigning Salmon Samurai Ordering Form
I work as a kitchen crew for Salmon Samurai, and I noticed customers usually miss out information on the forms. I too, have made many mistakes when I first joined and I thought, there has to be a better solution! From here, users involved aren’t just the customers, but also the cashier and the rest of the kitchen crew.
The Problem
The form requires the eye to scan all over the place, resulting in information often being overlooked (for both customers and crew). All the extra scanning requires time, higher accuracy and some getting used to so newcomers naturally make more mistakes which can disrupt the rhythm in the kitchen. Worst is when the form is filled with pencil scribbles.
The Challenge
The menu has to:
   •   be lucid and doesn’t feel overwhelming/bombarding
   •   in sequential steps so customers don’t miss things out
   •   require lesser scanning to speed things up and reduce error made

Format has to be the same
   •   black and white
   •   a5 size
The Solution
By asking customers the bowl sizes and food item at Step 1, I could easily identify the size of the takeaway box.

I had also eliminate the need to scan back up to identify the condiments by putting it under 'Add-Ons'. At the same time, also solving the dilemma of whether the customers truly don't want any or had failed to notice the section.
Process
Research
I first spoke to the cashier, as she not only understand how the kitchen runs, but also knows how customers react to the order forms.

Her input: Yes, customers often miss out condiment and it is incredibly time-wasting and vexing to keep asking them if they would like ginger or wasabi while multi-tasking. She also shared that some would write multiple orders on a form which creates problems for the kitchen crew as the form is designed to cater an order.
Sketches
As I have a few layout in mind, I sketch to visualize and think things through before putting everything on screen.
