OR: How I Became A Coward is a fiction/literary book. The concept of this cover shows the old style anatomy image of the spine which is the idea of 'courageous' (to have a spine), and also the idea of having guts. 

The question mark is in place of the stomach to show there are no guts, but it's a vacant place. The person/skeleton has no inner power or courage to call upon.
