The Lung Diagnostic Assessment Program (Lung DAP) approached Jessica Combate of Regional Printing and Graphics to design their logo and a basic identity package in late 2010. Lung DAP was created by the joint effort of two healthcare organizations in southern Ontario: St. Joseph's Healthcare Hamilton and Niagara Health System. The new Lung DAP logo would need to be representative of both entities.

The final logo represents a pair of stylized human lungs, fading from dark shades to light. The shades are divided as waves, representing the motion of air through the lungs. The colours grow lighter as they reach the base of the logo, where the name "LUNG DAP" provides support and strength.

The main colours for Lung DAP were borrowed from the respective identities of the parent organizations, St. Joseph's Healthcare Hamilton (left) and Niagara Health System (right). The gold colour (Pantone 4505) was taken from St. Joseph's secondary colour palette.
