The Problem

The appointment-booking form on Jun’s website is not aligned with the quality of his brand.
Future clients are left hesitant and unsure when they apply for a tattoo.

Hypothesis

Creating a more interactive, user-centered booking process will increase the number of high quality clients that join Jun’s waiting list.

___________

Interactive Demo Section 01

Before starting the application process, users are shown a disclaimer that not all applicants are accepted. They will have to acknowledge this before continuing to section 01 of the form where they get to choose where their tattoo will be.

Interactive Demo Section 02 / 03

This part of the form is a lot less visual for the user, making it a lot less engaging. So that the user doesn't feel overwhelmed with this "homework-like" task, the section is broken up into two smaller sections. Examples of entries are given, but tucked away to keep the design minimal and digestible.

Interactive Demo Section 04

In this final section, users upload reference photos for their tattoo application. The photos are then displayed with the option to attach descriptions to individual photos. This makes it easy for the user to communicate the specifics of the look they're trying to achieve.
