Objective
Create an intake form for Western’s Graphics Club.
Idea
I wanted to keep things organized and clean so it was easy to understand. I thought to make boxes with specific titles so it would be very clear as to what was being asked to fill out.
Execution
Using InDesign I created this form with Western’s colors to remind the user of who they are filling the form out for and to stay consistent with their brand identity. I also used color at the top and bottom of the page to keep the users eye focused on the information located in the center of the sheet.
