Problem Statement:
Patients needs to carry Hard copy of their past medical records for each visit to the doctor, the problem that has been faced by many patients in India. Owing to the physical nature of these records their are many instances where some patients forget important report. This missing information might puts the life of the patient at risk. All this is avoidable if provided a medical history by patient, dealing with any disease or accidental injury to a doctor.

Solution:
Designing a solution which is hassle free in maintaining medical records, existing as an information data for each and every user or stakeholder.

Focus for Contextual Inquiry at IIT Bombay Hospital
Limited to IIT and similar community based hospitals, when and why do users takes files out of the hospital and etc.....




Contextual Inquiry
Collecting work models (A graphical language that reveals patterns and structure of work.)
Artifacts, Culture, Sequence, Physical models  are five different perspectives that make the complexity of work comprehensible.
