Brief
It's that time of year again where we need to inform our staff of the upcoming Christmas Party. However since aquiring another company we will need someway of catering for requirements of staff at both offices.
We will have a Masquerade theme with black tie/evening dress. Can you design something that would show how the ladies and gents should look, please?
I currently have the following information available:
Date & Time: 12 Dec from 6pm till 1am
Venue: Terrace @ Wimbledon (above station)
Drinks: beer, wine, spirit and mixers, soft drink
Food: Prosecco & Canapés followed by Buffet dinner from 7.30pm
DJ from 9pm
I need to know who’s attending, so we will need a tick for that. If they tick yes then some questions should appear:

Final
Includes a Google map link on 'Terrace @ Wimbledon' which takes the staff member to a map with a dropped pin at the venue location. This is important as staff from the branch office that are unfamiliar with the area will be able to view the map before arriving and when viewed on a mobile the map will navigate them directly to the venue. Also included is a link to videos on the 'Live Band's' website which creates excitement for the event.
UX Tweaks
We have a list of staff memebers and with that we have access to names and where that staff member is based. With this knowledge we can streamline the form further and remove any unnecessary information.
Pre-populate the 'Name' field and If the recipient is based at the main office, which is close to the venue, do not show the option of coach travel.
