As depicted, my setup included:
- my D90 tripod-mounted, aiming down at my 'storybook' and set to remote
- 'storybook' at base of table with brush pens and pencils to hand
- my storyboard portfolio (between tripod legs) for reference
- one desk lamp 
- shit-loads of Blu Tack to keep the story book down 
- one 8 hour shift
- a few JD & Cokes (it was Christmas!)
