The Patient Engagement mobile app is about the tracking of patient's health records and record their tracking data's at their finger tips. Created this UI design by bringing the graphical view of representation in patient's dashboard page.

Need an user interface where the user should track their personal health record at their finger tip and should be able to track all their histories of health data.
