A US birth certificate is a government-issued document that officially registers a child's birth in the United States of America. However, there is no national birth register, only a standard form or guidelines of the information.

Therefore, each state, county, or municipality produces birth certificate documents for its citizens with varying formats. They have some freedom as long as they follow federal guidelines on the information printed on their birth certificates.

According to the National Center for Health Statistics, there are 14,000 different birth certificates currently circulating in the United States.
