Patrons at our animal shelter were complaining because we transfer in so many pets from other overburdened shelters that we don't have much information about since they likely came into the previous shelter as a stray, so their history is a mystery. This idea allowed volunteers and staff to mark down what they do know about the pets during their stay at the shelter, which helped potential adopters to have a better idea if our dogs and cats would be a good match for them.
I came up with this idea based on what I've seen other shelters do, which is just a really basic report card. I came up with the options for volunteers to mark based on what I've learned in my animal behavior classes.
