Heat Map Bubble Diagram

The task here was to represent 3 axis of information in a single diagram view to follow the ascending and descending risk of patients to be readmitted into the hospital within 30 days of discharge. Each dot represents a different patient the hospital is currently monitoring. This was a concept UI for a population health monitoring program.
 
The 3 axis are:
Time - The dots move from left to right, starting during a hospital stay and measuring readmittance risk to 30 days after initial discharge
Risk - The dots move up and down between low and high risk zones as time passes.
Growth and Decrease of Risk over Time - The increasing diameter of a dot over time represents a patient that is approaching a higher risk for readmission. The decreasing diameter of a dot over time represented a shrinking risk of readmission.
The slider at the bottom of the screen can be used to trace a patients path through the risk zones. The data could be used to look for patterns among patients, or to correlate risk levels to care or patient behaviors at a given time.
The top third of the display shows the total number of patients monitored and what time period of their stay they are in. The bar chart represents the readmission trend over the last 30 days of monitoring.
Basic patient information could be opened by rolling over a dot. Individual dots or clusters of dots could be selected and displayed in a list view.
