Salud por Vida/Health for Life
Salud por Vida/Health for Life was an initiative designed to address disparities in diabetes education, particularly in the Hispanic community. Working in El Paso and the Rio Grande Valley, we collaborated with local organizations and diabetes educators to increase access to diabetes education classes and encourage attendance. We provided training materials and education for providers and information, tools and awareness campaigns targeted at patients.

This card was part of a kit containing a pedometer, exercise bands and a squeeze ball that was distributed to partners and physicians who would then give them to their diabetes patients.
