This is a personal project to redesign the forms at a local veterinarian clinic where I live to make them more usable, readable, and just more organized as a whole. I also wanted to make them took relatively the same and like they all belong together.
Pet Heath Questionnaire Key Changes:
1. I made sure all like information gathering was together. Health related questions towards the bottom, personal questions at the top.
2. I made sure all Information gathering was the same throughout- now you would always be checking the circle for whatever you are agreeing too or writing on a line.
3. Using my typography to build hierarchy better and help clients read and use the forms faster.
4. The light blue boxes in the background allow for these information gathering sections to stand out against one another, so that you do not accidentally get the lines confused.
5. typography is larger and heavier throughout.
a lot of the clients who come into these clinics are older, so making it easy to read with larger typography made sense to me, even if I typically would not do that. I wanted this to be usable easily for clients who may not read well. I also changed some sentence structures or wording due to awkwardness.
Pet annual Exam Key Changes:
1. added check circles so that clients can easily add on additional treatments to their exam.
2. Once again, Used typography to build hierarchy- making it easier to read throughout.
3. adding a "No" option at the bottom.
without these changes clients would often be confused, not know what to sign or not sign or how to add on the additional services, and would hand back a blank form which isn't helpful for the staff.
New Client Form Key Changes:
1. Making enough room on form to write information in.
2. moving signature, highly important things, to the bottom of the page so it does not get missed.
3. making type larger and heavier, for elderly clients to make it easier for them to read.
4. fixing minor formatting issues that just made things a little awkward overall.
